Covered securities, broadly those acquired after basis reporting phase-in dates, have basis reported to the IRS. Non-covered ones show proceeds only, and you supply the basis, which is why old transferred holdings need your own records.
The wash sale figure shown is calculated within that account for identical CUSIPs only. It does not see your other brokers, your IRA, or substantially-identical-securities such as options on the same underlying, so the correct number on your return can legitimately differ from the form.
Futures and other section-1256 positions appear as an aggregate profit or loss figure rather than trade by trade, feeding form-6781 instead of form-8949.
